Voter Registration
Steps:
- Click on the Voter Registration Application (EB-131) link below and complete the entire fillable application by carefully following all of the instructions on page 2 of the application.
- Print the completed Voter Registration Application (EB-131) and mail or hand-deliver to:

- **VERY IMPORTANT**
- If you are registering by mail, the application must be postmarked no later than the 20th day (3rd Wednesday) before the election (For the September Partisan Primary, that date is Wednesday, August 20, 2008, and for the November Presidential Election that date is Wednesday, October 15, 2008) or you will be required to show valid proof of residence (acceptable documents shown below) at the polling place on Election Day.
- If you are registering in person after the 20th day before the election as outlined above, you will be required to show a valid proof of residence (acceptable documents shown below) at the Clerk/Treasurer's office.
- If you are registering to vote by mail for the first time in Wisconsin or for the first time since 2000, you are required to send a photocopy of a valid proof of residence (acceptable documents shown below) with application.

Proof of Residence
The following constitute acceptable Proof-of-Residence if the document contains:
A. A current and complete name, including both the given and family name; and
B. A current and complete residential address, including a numbered street address, if any, and the name of the municipality.
- A current and valid Wisconsin driver's license.
- A current and valid Wisconsin identification card.
- Any other official identification card or license issued by a Wisconsin governmental body or unit.
- Any identification card issued by an employer in the normal course of business and bearing a photo of the card holder, but not including a business card.
- A real estate tax bill or receipt for the current year or the year preceding the date of the election.
- A residential lease which is effective for a period that includes Election Day (NOT for first-time voters registering by mail).
- A university, college or technical institute fee card (must include photo).
- A university, college or technical institute identification card (must include photo).
- A gas, electric or telephone service statement (utility bill) for the period commencing not earlier than 90 days before Election Day.
- Bank statement.
- Paycheck.
- A check or other document issued by a unit of government.
REQUESTING AN ABSENTEE BALLOT
Steps:
- You must first be registered to vote in the City of Fort Atkinson before you are eligible to request an Absentee Ballot. Please see instructions for registering to vote above.
- Click on the Application for Absentee Ballot (EB-121) link below and complete the entire fillable application by carefully following all of the instructions on page 2 of the application.
- Print the completed Application for Absentee Ballot (EB-121) and hand-deliver or mail to:

- **VERY IMPORTANT**
- If you are applying by mail, the final day a request may be received is October 30, 2008 for the upcoming November 4, 2008 General Election.
- If you are applying in person at the Clerk's Office you may request and vote until 5:00 p.m. November 3, 2008.

Volunteer at the Polls
The City of Fort Atkinson is also looking for poll workers. Poll Workers need to be residents of the City, be available to work on election day (either morning shift or afternoon shift), meals will be provided, will be paid an hourly wage, and may be asked to take 3-6 hours of training locally.
